The Agency provides home health social worker services to Medicare, Medicaid, private insurance and private pay adult patients within the Agency’s service area that require skilled therapy, with the following goals:
a. To ensure the patient’s receive quality, comprehensive social worker services from highly qualified, dedicated, proficient and caring social work personnel.
b. To promote realistic patient outcomes through coordination of care, treatment and/or services based on patient needs and clinical status.
c. To maintain effective channels of communication with the patient, physician, and other organizations and providers within the health care community.
d. To provide the patient and his/her family with resources to meet his/her needs and education to enhance healing in the home.
